By ticking the box “I give my consent to the processing of personal data” and submitting the form on bxk.ru, I freely, of my own will and in my own interest, give Joint Stock Company “Baltic Chemical Company” (JSC “BHK”), INN 7811458644, OGRN 1107847075060, address: 193318, St. Petersburg, Voroshilova St., 2, lit. A, premises 5-N (the “Operator”), my consent to the processing of my personal data on the following terms (Article 9 of Federal Law No. 152-FZ).
1. Purposes of processing. Reviewing my request, contacting me about the request, preparing a cost estimate and a commercial proposal.
2. Personal data. Name, telephone number, e-mail address, company name, information provided by me in the comment and attached files; the list of website pages viewed during the visit in which the request was submitted and the referral source (unless I declined cookies).
3. Actions with the data. Collection, recording, systematisation, accumulation, storage, clarification (updating, modification), retrieval, use, blocking, deletion, destruction — with or without automation tools. The data are stored on behalf of the Operator by a hosting provider on servers located in the Russian Federation. The data are not transferred to any other third parties, except as required by law.
4. Term and withdrawal. The consent is valid until the purposes of processing are achieved, but no longer than 1 year from the date of the last contact. The consent may be withdrawn by a request sent to info@bxk.ru or to the Operator’s address. Upon withdrawal, the Operator stops processing and destroys the data within 30 days, unless otherwise provided by law.
5. Confirmation. I confirm that I have read the Operator’s Personal Data Processing Policy. This consent is specific, informed, conscious and unambiguous (Part 1, Article 9 of Law No. 152-FZ).
